MSSP’s goal is to help the frail elderly live an independent life of dignity at home by delaying or preventing institutional placement. MSSP provides a caring and qualified Case Manager who will meet with you to learn your needs for independent home living. A Public Health Nurse and a Case manager will provide each client with a complete health and psychosocial assessment to determine the services needed. The team then works with the client and family to develop an individual care plan that will draw on existing community resources or services purchased for you. Services must be provided at a cost lower than that for nursing home placement. Your MSSP Case Manager will arrange for the assistance that you need and will check regularly to make sure these services continue to meet your needs. When your circumstances change, you Case Manager will update your plan services to meet your needs.
